How do I apologize to a co-worker for a BAD mistake I made?

I was supposed to go help a co-worker get her computer set up for her to participate in a teleconference. I forgot about it!! I was away from my desk (where the reminder popped up) and I just forgot all about it. She got kicked out of the conference because her microphone wouldn’t work and now she can hear (but not see or participate in) the teleconference. She is very upset with me, as I am with myself. The teleconference is VERY IMPORTANT to her. I just feel awful. Any suggestions?
